Tampa, FL (March 22, 2012) -- Immediate skull reconstruction following trauma that penetrates or creates an indentation in the skull can aggravate brain damage inflicted by the initial injury, a study by a University of South Florida research team reports. Using a rat model for moderate and severe traumatic brain injury, the researchers also showed that a delay of just two days in the surgical repair of skull defects resulted in significantly less brain swelling and damage.

The study was published March 16, 2012 in the online journal PloS ONE. While further investigation is needed, the findings have implications for the acute treatment of traumatic brain injury (TBI), considered the signature wound of soldiers who have served in Iraq and Afghanistan, said the study's principal investigator Cesar Borlongan, PhD, professor and vice chair of research at the USF Health Department of Neurosurgery and Brain Repair

"A double-edged sword," is how Borlongan describes the inflammation and subsequent swelling of brain tissue that occurs immediately following TBI.

When the brain is initially penetrated -- by a bullet, shrapnel, other debris, or even the force of blast waves, for instance -- inflammation helps recruit the body's own good (glial) cells to the damaged site to limit localized injury. For a short time, the inflammation-induced edema, or swelling of the brain, is beneficial to help relieve pressure within the skull. However, chronic inflammation precipitates increases in intracranial pressure that perpetuate a vicious cycle leading to secondary cell injury and death.

Cranioplasty is an operation to repair malformations of the skull caused by TBI; the procedure may involve replacing a missing piece of the skull protecting the underlying brain and/or improving the appearance of the skull's surface. Current clinical practice emphasizes performing cranioplasty quickly upon initial hospital admission to help reduce the likelihood of infection or other complications that may arise when the brain is exposed.

"Our preclinical study indicates that reconstructing the skull too early in the brain's natural healing process may interfere with critical therapeutic benefits of brain swelling post-TBI," Dr. Borlongan said. "It's better to wait at least two days."

The USF researchers studied rats with moderate and severe TBI. Post-TBI, the animals were randomly assigned to skull bone flap replacement with or without bone wax (a sterile mixture to help control bleeding from bone surfaces); no skull reconstruction; or delayed skull reconstruction with bone wax alone, which was performed two days following TBI.

The brains of all the animals were analyzed in the laboratory five days after surgery. While immediate reconstruction provided aesthetic repair of the skull fracture, this early surgical procedure, with bone wax alone or with bone wax and skull bone flap, significantly increased cortical brain tissue damage in both moderate and severe animal models.

Overall, whether the rat model was moderate or severe TBI, delayed reconstruction limited the worsening of brain tissue damage compared to immediate reconstruction. In fact, for moderate TBI, the extent of damage observed in the brains of rats that received delayed reconstruction was on a par with that in the animals getting no reconstruction. In those with severe traumatic brain injury, the tissue damage was significantly larger. The authors suggest this may mean a two-day delay, while more beneficial than immediate reconstruction, was not sufficient to counteract the intracranial pressure generated by severe TBI.

The researchers concluded that the timing of cranioplasty warrants further evaluation in both laboratory and clinical settings.

"Our results suggest that delaying cranioplasty until the TBI-induced cerebral swelling has subsided may reduce unwanted exacerbation of cortical damage associated with skull reconstruction," Borlongan said. "We need to carefully weigh the risk of infection that comes from leaving the brain somewhat exposed with the benefit of enhancing the brain's own repair of its cells."

"Finding a safe and effective cranioplasty regimen will require determining the optimal period of time when we let the brain repair itself and balancing that with when to best introduce a regimen of surgical skull repair and other potential therapies," said co-author Harry van Loveren, MD, the David W. Cahill endowed professor and chair of the USF Health Department of Neurosurgery and Brain Repair.

This is Mariusz Pudzianowski, he's (I think) a five time World Strongest Man, he's a strongman competition competitor and MMA fighter. On one of the World Stongman events shown on TV, as well as in an interview for MTV, Mariusz when asked about diet said: "I eat everything. I do not follow any particular diet. I eat anything I want, anytime I want".[28]

Anyway, yeah, regarding the diet, though, breaks all the rules, and, well.... look at the results. One thing I thought of in regard to this was the Lewis Black routine, the one about the guy who lived to be 115 eating bread fried in fatback and Thunderbird wine (which is an exaggerated story.) Here's the skit I'm talking about, btw...

Anyone else have any thoughts about his diet? Nuts? Awesome?

(AP) ? Few people want to be wide awake during their colonoscopy exams, but new research suggests too many are getting extra sedation treatment, costing as much as $1 billion yearly in potentially needless services.

Use of anesthesiologists to monitor sedation during colonoscopies and other digestive imaging tests has more than doubled in recent years, and they're used most often for low-risk patients who typically don't need the extra help, the study authors said.

"These services are not harming patients. They're basically giving them a luxury that is not strictly necessary," said the study's lead author, Dr. Soeren Mattke, a senior Rand Corp. scientist. That matters at a time when policymakers are trying to rein in rising medical costs, the authors said.

Patients usually are briefly sedated for a colonoscopy, and some kinds of sedation require monitoring by specialists. That includes use of propofol, a powerful intravenous drug that can cause deep sedation and was implicated in pop star Michael Jackson's death. Anesthesiologist-monitored sedation, with or without propofol, is recommended for high-risk patients, including those who are old or sick or previously had complications with anesthesia.

While propofol sedation is also sometimes given to low-risk colonoscopy patients, the study authors suggest that's often unnecessary. Drugs usually recommended for these patients typically cause lighter sedation, though most patients don't remember anything about the exams afterward. These drugs can be given by the doctor doing the exam, but the study suggests they're often also being given and monitored by anesthesiologists.

The researchers analyzed insurance claims data on more than 6 million U.S. adults who had the colon exams or imaging scopes of the upper digestive tract between 2003 and 2009. The tests are done to screen for colon cancer, acid reflux and other illnesses.

When the study began, 14 percent of these tests included an anesthesiologist. That climbed to more than 30 percent by 2009. The portion of this extra sedation treatment given to low-risk commercially insured patients remained constant during the study and fell slightly in Medicare fee-for-service patients. But the study authors said far too many low-risk patients are still getting this treatment ? more than three-fourths of the commercially insured patients and two-thirds of the Medicare group.

The extra treatment added an average of about $500 to an insured patient's bill in 2009, and $150 to a Medicare bill. In 2009, about 3 million colonoscopies and other digestive scoping tests were done in low-risk patients but included anesthesia services, amounting to $1 billion in potentially unnecessary costs, the study authors estimated.

The study appears in Wednesday's Journal of the American Medical Association.

While some insurance policies exclude coverage for anesthesiologist monitoring for low-risk patients undergoing these exams, insurers sometimes pay for it, Mattke said.

Because of doctor backlash, Aetna, one of the nation's largest health insurers, has indefinitely delayed a policy it tried to implement in 2008 that would have excluded this coverage for low-risk patients, said Dr. James Cross, Aetna's chief of national medical policy and operations.

Reasons for the frequent use of anesthesiologists during these exams vary. Sometimes low-risk patients undergoing colonoscopies and other digestive scoping exams request propofol because they want to be totally unconscious and have heard that it wears off quickly and doesn't cause grogginess, unlike other sedatives, said Dr. John Vargo, a spokesman for the American Society for Gastrointestinal Endoscopy and a digestive specialist at the Cleveland Clinic.

Propofol requires careful monitoring because it has "a narrow window between providing deep sedation and making people stop breathing," and unlike other sedatives, there's no rescue drug to reverse its effects, said Dr. Norm Cohen, vice president for professional affairs at the American Society of Anesthesiologists.

The rising use of anesthesiologists may partly reflect more obese patients and users of Vicodin and other opiate-based prescription drugs, Cohen said. Both may be missed under coding used in the study, but they should be considered at risk because sleep apnea that often accompanies obesity makes sedation trickier, and users of opiate painkillers often require higher than usual doses of sedation, he said.

Vargo said doctors who do the exams can be trained to use propofol in healthy patients, but a journal editorial said some prefer anesthesiologist assistance because it allows them to focus on the colon exam, and if something goes wrong with the sedation, they may not be held legally accountable.

6waves Lolapps Sheds Most Of Its Development Team To Focus on Publishing Games

6L-Logo_v26waves Lolapps, the social gaming company that was formed out of a merger just half a year ago, shed most of its employees on the development side today to focus on publishing other studios' titles. The decision today seems to undo some of what that merger agreement created last July and we hear it had to do with a limited runway of cash. Last summer, social gaming company Lolapps merged with game publisher 6waves to form a new entity that was a dual producer and publisher. They then raised funding from Korean gaming giant Nexon and Insight Venture Partners. The amount they raised was undisclosed, but an SEC filing showed a $35 million round. The company insists, however, that the merger is still in place and Lolapps is not being spun out. They didn't disclose how many people were laid off.

Christians gather to bid farewell to Egypt's Pope

CAIRO (Reuters) - Christians paid final respects on Sunday to Pope Shenouda III, who spent the last months of his four decades at the head of Egypt's Orthodox Church trying to soothe sectarian tensions that have escalated since the ouster of President Hosni Mubarak.

Ten of thousands queued at the cathedral where Shenouda's body was initially laid in a coffin and later seated on a ceremonial throne wearing gold and red embroidered religious vestments, a golden mitre on his head and holding a gold-topped staff.

The lines blocked traffic in central Cairo and some women and elderly fainted in the heat. The cathedral opened and closed its doors several times in an attempt to contain the crowds.

Two people were killed in the crowding, medical sources said. One suffered a heart attack and the other, an elderly citizen, died of exhaustion.

Mubarak, who suppressed Islamists, was ousted last year. Since then Shenouda, who died on Saturday aged 88, often called for harmony and regularly met Muslim and other leaders.

Christians, who comprise about a tenth of Egypt's 80 million people, have long complained of discrimination and in the past year stepped up protests, which included calls for new rules that would make it as easy to build a church as a mosque.

Christians have accused hardline Islamists of attacking churches and said the authorities have failed to step in to protect them, although experts say some recent incidents have been fuelled by local grudges as well as sectarian tensions.

Shenouda had served as the 117th Pope of Alexandria since November 1971, leading the Orthodox community who make up most of Egypt's Christians. His funeral will be held on Tuesday, Egyptian state media reported.

CONDOLENCES OFFERED

Interim military rulers paid tribute to the church, visiting the grounds of the Orthodox Coptic Cathedral in Cairo. Field Marshal Mohamed Hussein Tantawi who Mubarak handed power to promised to air lift Shenouda's body to where he will be buried.

U.S. President Barack Obama offered his condolences and Pope Benedict, leader of the world's Roman Catholics, offered prayers after being informed of his death.

"I would like to express to the members of the Holy Synod, to the priests and to the faithful of the Patriarchy, my strongest feelings of fraternal compassion," said Benedict.

Describing Shenouda as a long-time advocate of unity among Christians, he said the Catholic Church "shares the pain afflicting Orthodox Copts."

Shenouda was popular among many of Egypt's Christians even outside the Orthodox Church, as well as among many Muslims. However, some Christian activists said Shenouda should have pushed the state harder to secure more rights for Christians.

In one phrase Shenouda often repeated and which was also cited in newspapers on Sunday, he would say: "Egypt is not a nation we live in, rather it is a nation that lives in us."

The burial is expected to take place at the Wadi el Natrun monastery in the desert northwest of Cairo, where the late pope had requested he be buried.

Shenouda was banished to Wadi el Natrun monastery in 1981 by then-President Anwar Sadat after he criticized the government's handling of an Islamic insurgency in the 1970s and Egypt's 1979 peace treaty with Israel.

Under Mubarak's rule, relations between the government and the Coptic church were generally smooth, with the pope portrayed in state media as a symbol of religious harmony, despite occasional outbreaks of sectarian violence.

RELIGIOUS DIALOGUE

"We will remember Pope Shenouda III as a man of deep faith, a leader of a great faith, and an advocate for unity and reconciliation," the U.S. president said in a statement issued by the White House.

Obama said Shenouda had been committed to national unity and was "a beloved leader of Egypt's Coptic Christians and an advocate for tolerance and religious dialogue."

Shenouda publicly supported Mubarak during his last days and before Mubarak's ouster by a popular uprising on February 2011, a move that drew some criticism from some members of his church who joined the protests that ousted the president.

Some Muslim leaders also backed Mubarak in his last days.

Bishop Bakhomious, head of the church of Bahaira, a district in the Nile Delta north of Cairo, will temporally hold the post of pope for two months until a new leader is elected.

Egyptian media described the procedure for choosing a new pope as one based on a system of voting by board members of the church's city councils. The councils vote on three preferred candidates, and the final choice is made when a name is picked out of a box by a young child, the media said.

I think it?s important we look at the foundations that need to be laid. Just like the footings of a building, to become successful you need to put down good foundations. To determine which foundations are most important, I reviewed the BRW Rich List 200 and Young Rich List to find out what the people who?ve done the best job in the country at gaining wealth have done to get where they are.?

Here?s what I found those people have used as the foundations of building their fortunes:?

  1. They spend less than they earn.
  2. They set aside a certain percentage of their profits or wages to invest.
  3. They set goals and focus on achieving them.
  4. They create a team of advisors that can guide them and provide them with the information they need.
  5. They aren?t afraid to take calculated risks.
  6. They persist. They never give up. They remove words and phrases such as ?I can?t?, ?It won?t work?, ?It?s hopeless?, ?I quit? and ?It?s impossible? from their vocabularies.
  7. They use leverage or other people's money for growth and they use time and growth as their friends. They invest with a buy-and-hold mindset and aim never to sell.
  8. They always run in the opposite direction from the crowd. They invest counter cyclically. As human beings we tend to follow what everyone else is doing, but those few that invest against the crowd are the people who always make it to the summit.?

Unfortunately most of us never make it past the first of the foundations on that list. Of those who do, many stumble at number 5 because of the fear that sets in when it comes to taking risks.?

Wealth creation is not about emotions. It?s about removing all your emotions and focusing on the deal, looking at the research, doing your number crunching and talking it through with your team.?

If you want to make yourself wealthy you have to be willing to make sacrifices at the beginning. You have to be willing to not spend your surplus income, but to put it aside and slowly watch your savings grow until you have enough to invest.?

Patience and persistence are needed in order for your funds to grow and then your investments to grow. Many people buy a single investment property and aren?t happy to find they?re not immediate millionaires, so sell their investment too soon. That?s just not how wealth building works.?

Remember slow and steady, knowledgeable decisions and calculated risks will get you there, and when you sense fear, turn to your team for the push you need.?

US set for China rare earth case

The US is set to file a case against China at the World Trade Organization challenging its restrictions on rare earth exports.

The European Union and Japan are expected to support the US in its case.

Beijing has set quotas for rare earth exports, many of which are critical to the manufacture of high-tech products.

The US argues that by limiting exports China, the world's largest rare earth producer, has pushed up prices.

China produces more than 95% of the world's rare earth metals, and any restriction on exports from Beijing is likely to have a bearing on global prices.

There have also been concerns that Beijing has implemented these quotas in a bid to ensure that prices of these elements remain low within China, a move that critics say gives its manufacturers an unfair advantage.

However, China has denied these allegations, saying that it had enforced these quotas to ensure there was no environmental damage caused due to excessive mining.

The US administration is likely to file the case on Tuesday according to Reuters, AFP and the Associated Press news agencies.

    • Select 'Internet Options' from the 'Tools' menu in your browser window

    • Adjust your privacy setting if necessary

    • Open Internet Explorer
    • Select 'Internet Options' from the 'Tools' menu in your browser window
    • Click the 'Programs' tab and check the box for Internet Explorer to check if it is the default browser and save your change
    • Close your browser, re-open it, and when prompted, select Internet Explorer as your default
    • You can then click on an ad in your newsletter and visit the site you wish to view
